Click here to apply via Benefits.Ohio.gov, use your Self-Service Portal account, or access the paper application titled “SNAP, Cash, Medical and/or Child Care Assistance application.
If choosing to use the paper application, print out, answer all questions, and sign the paper application. Then submit that completed application using one of the following options.

This program provides fuel reimbursement, public transit reimbursement, and point-to-point transportation through a van service to customers actively approved for Medicaid covered services and for work-related purposes only. There may be a fee involved for work-related service transportation services. For more information, call 937-327-1710.
