When calling to file application please have the following available:
- Social Security Number, Driver License, or state ID.
- Name, address, telephone number, and dates of employment.
- For each employer you worked for in the last 18 months.
- If your separation is part of a mass layoff or buyout, the ID number provided by your employer.
- Your alien identification number, if applicable.
- Your dependants name(s), birth date(s) and social security number(s).
- If interested in Direct Deposit, your bank account and routing number.
- Your child support court order or SETS number (OH only), if applicable.
